Step by Step Process to join us on an Olivia Ray House build

Step One – click on the house build application form and print using your browser print button

Step Two – click on each form below and print using your browser print button
◊ Minor Liability (one per person)
◊ Adult Liability (one per person)
◊ Minor Treatment consent (2 copies, one to be sent in, one for you to bring with you)
◊ Adult Treatment Consent (2 copies, one to be sent in, one for you to bring with you)
◊ Affidavit of sole custody notarized (2 copies, both notarized, one sent in, one to bring with you)
◊ Make a copy of your passport page with your photo and information
◊ Include a check for $75 (registration) made out to:
Homes of Hope with a notation for Olivia’s House Build

Step Three – Send everything to:
YWAM San Diego/Baja
Attn: Homes of Hope Registrar
100 West 35th Street Suite C
National City, CA 91950

Step Four – earn/beg/car wash/ treasure hunt/ask your grandparents for the
$220 in fees and additional costs for airfare for the build
Step Five – book your plain tickets for your arrival to SanDiego
Step Six – Send in your $220 in final fees by June 7
Step Seven – pack old shoes, old tee shirts, old jeans, bathing suit, pillow and
bedding, sunscreen, a towel, soap and bathing suit (oh yeah mentioned that)
Step Eight – Head out and join the Ray’s and a bunch of other people to build a cool house
